Course Authorization Forms

Class permission can ONLY be granted if a student has completed ALL of the prerequisites for the course OR is currently enrolled in the prerequisites/corequisites. 

If students haven't completed the prerequisites or are not currently enrolled in the prerequisites/corequisites, they will not be granted the class permission.

Please use the form below to request permission for courses in the Fairbanks School of Public Health.

The honors authorization form may be used for any honors courses, except for
honors chemistry, SCI-I 120, or business. Please use science or Kelley
authorization forms for those specific courses.

The authorization site below is restricted to advisors who have been approved for access. To request access, please submit an email to advising@iupui.edu. 

Course authorization for Bridge Week, learning communities, and FYS (UCOL-U 110) courses

If a student needs to drop one of their learning community courses, they must drop all courses within the learning community. To do so, advisors should complete the learning community drop request form. Note that you do not need to use this form until the blocks break, typically 10 days before the start of the semester.Prior to that time, students can simply drop the learning community block number (e.g., UCOL-BE 499).
